When you submit the contact form, your name, email address, country or region and enquiry topic are sent to [email protected]. We use them to respond to your question and arrange the next steps with you. You can also email us directly.

Start with the information needed to explain your situation. Before you share complete medical records, we agree which documents are needed, who will review them and how they will be shared.

Let your coordinator know who you would like included in the conversation. We ask for your permission before sharing your case with family members or a doctor at home.

Cloudflare hosts the website, checks submissions for automated abuse using Turnstile and sends form enquiries to our Zoho Mail inbox. The website keeps a submission reference, a digital fingerprint and delivery status for up to eight days to prevent duplicate emails; it does not keep a separate copy of your message or contact details. Email correspondence is stored in our mailbox. These providers may process information outside your country under their privacy terms.

Before you entrust us with your records, we explain who is responsible for them, how they will be used, who will receive them and how long they will be kept. You can raise questions about access, corrections or deletion with your coordinator, or email [email protected].
